The Companion Card Cardholder Handbook is available in 15 languages, and they include information about the Cardholder Application Form. There is also Recorded Information provided in 15 different languages.

English Translation of Language Pages

You can obtain written and spoken information about the Companion Card in the following ways;

Download documents:

  • You can download the Companion Card Cardholder Handbook (PDF, xxKB, xxpages), which provides information about the Companion Card Program, including who is eligible and the terms and conditions of using a Companion Card. This document outlines the information required to complete the Cardholder Application Form. This Handbook is provided as an Adobe Portable Document Format (PDF). To access this document you will need the program Adobe Reader. Often this is already installed on your computer; otherwise you will need to download it from the Adobe web site (http://www.adobe.com/products/acrobat/readstep2.html). If you have any trouble with downloading any attachment, please email: Companion Card webmaster@dpcd.vic.gov.au or see below for alternatives ways of obtaining the documents.
  • You can download the Companion Card Cardholder Application Form (PDF, xxKB, xx Pages). If you wish to apply for a Companion Card, you must print and complete this Application Form. This form is written in English and must be completed in English. This Handbook is provided in an Adobe PDF format.

  • If the information you require has not been covered in the handbook or by the recorded information line, the Companion Card Information Line staff may be able to assist you with your query. Telephone the recorded information line and follow the instructions to connect directly with a speaking interpreter who will assist you to speak with the Companion Card Information Line.
  • If the person telephoning is English-speaking, the best and simplest way for them to contact the Companion Card Information Line directly is to telephone 1800 650 611.
